The ismv file extension is mainly related to Microsoft's Smooth Streaming service. Smooth Streaming is extension for Internet Information Services (IIS) and used to streaming video in to Silverlight and other compatible clients.
The ismv file contains specially fragmented MPEG-4 multimedia content in Smooth Streaming format (also can use a PIFF - Protected Interoperable File Format).
Specialized plugins can be found that add support for *.ismv files to web browsers. The easiest option would be to use tools provided by Microsoft.
There are known problems related to conversion of .ismv file types. Some users had luck with using Windows Live Movie Maker (now known as Movie Maker), whilst for others it did not actually work. Some video converters claim to support this file format but this may be just marketing trick.
