FEFLOW, developed by DHI, is professional software for modeling fluid flow, the transport of dissolved constituents, and heat transport in the subsurface. The fem file is the main problem file used by the software.
A .fem file can contain model-related information, including Problem Settings, parameters, and View window settings. It can be stored in binary or ASCII text form. The .fem file is the only file needed to run the model.
Maps are stored as references to the original map files, using paths relative to the location of the .fem file. FEFLOW is available for Microsoft Windows.
Open the .fem file in FEFLOW on Microsoft Windows. In normal use, FEFLOW reads the file and its referenced map files as part of the model.
The file is not intended to be edited manually. An ASCII .fem file can be viewed as text, but changing it outside FEFLOW may make the model unusable.
It does not seem likely that FEFLOW .fem files can be converted or exported to another format.

File extension fem is used for one of the native file types of NX, a high-end CAD/CAM/CAE software by Siemens.
The FEM file stores all finite element data (nodes, elements, connections, material properties, element properties) for a specific entity. The finite element data is, by default, associated to the geometry of the Idealized Part or Part (.prt files) used for construction of the mesh.

Whole parts (.prt) can be exported with the data provided by (.fem) files.
