File extension fem seems to be associated with FEFLOW, a professional software package for modeling fluid flow and transport of dissolved constituents and/or heat transport processes in the subsurface.
A .fem file is used to store all model-related information, such as the Problem Settings, the Parameters and View window settings. Only this file is needed to run the model. Maps are stored as references to the original map files relative to the location of the *.fem file. These files can be stored either in binary mode or as ASCII (text) file.
As far as we could gather, this file type is only used in the original FEFLOW and is not meant to be accessed by the user manually.
It does not seem likely that .fem files from FEFLOW can be converted or exported to anything else.

File extension fem is used for one of the native file types of NX, a high-end CAD/CAM/CAE software by Siemens.
The FEM file stores all finite element data (nodes, elements, connections, material properties, element properties) for a specific entity. The finite element data is, by default, associated to the geometry of the Idealized Part or Part (.prt files) used for construction of the mesh.
Can be opened/loaded along .prt file in Siemens software.
Whole parts (.prt) can be exported with the data provided by (.fem) files.
